    Instructions
    • In case of loss of membership card, member should immediately inform the Club’s management on telephone and lodge a Police FIR to avoid its misuse.
    • For duplicate membership card FIR copy is required to be uploaded in case card is lost.
    • In case card is damaged, picture of damaged cards will be uploaded.
    • Only JPEG files to be uploaded (Where required).
    • Size per attachment must not be larger than 1 MB.
    • Cards will be issued after 8-10 working days after submission of this form.
    • Member / household members or authorized persons, may collect membership cards during working hours from club’s office.
    • Applicable fee will be charged in monthly bill.
    • All the Bye Laws of respective Club will be applicable.
